It used to be that the 4th statline was invisible. A QOL update at the beginning of 6.0 changed the system so that the 4th statline was visible, but "deactivated".
Leveling it to 4 will always go towards activating that statline. The remaining 4 upgrades will go to random stats as usual.
This is why 4-line Artifacts are preferable when min-maxing, as you get one extra upgrade out of it, instead of it being wasted on the unlock.

Substats are sometimes locked, RNG based. You'll get it when you level it to +4
(4 starting lines vs 3 starting lines is really just another layer of rng to the artifact system, because artifacts get upgrades at +4, 8, 12, 16, 20. If one substat is locked, getting it to +4 will unlock it, resulting in one less upgrade than a 4-liner. In total, a 3 liner will have 8 substats and a 4 liner will have 9 in total. )
