[Spoilers] An answer I think we never got

Why exactly did Nefer see Tholindis's memories in Rerir's past? I remember there being a theory that after Rerir shattered he mixed with some pieces of Tholindis, but now we know that they are both stuck in The Moon's Reflection and will never be able to even meet.

Did I miss something here?

I still think bits of Tholindis' body got mixed up with Rerir. It is after all only her soul that is stuck in the Moon's Reflection.

Correct, this was never clarified. There is no in-game explanation as to why Nefer was seeing Tholindis's memories along with Rerir's.

He said that some fragments of her soul, some of which were in the Moon Gate, were here. Is it possible that fragments of her soul were thrown to different places, not just the Moon Gate?

First I want to clarify what Rerir meant when he said that it's his second time in moon gate. If he really is then he borrowed a piece of Tolindis the first time. If his first time he meant being the Wild Hunt the last 500 years then dunno
