The talk about 6th seat being vacant for hundreds of years and questioning it only appears after Wanderer removed himself from Irminsul. It was easier for World Tree to just wipe memory of him than create fake memories in the past where there was actually 6th Harbinger(s) or questions why it was vacant. Only after Wanderer did the thing, they could start asking questions why 6th seat is vacant for so long.
Of all people, I would argue that Pierro might be realize of Irminsul shenanigans. After all he is Director of the Fatui and he would ponder in present time why he never assigned someone the 6th seat and more importantly, why he only started wondering about it in present time, not hundreds of years ago.
This is not like it was with Nahida and >!Rukkhadevata!<, where Irminsul replaced >!Rukkhadevata!< with Nahida since they were almost same entity and made explanation that she is in such form because she sacrificed her powers, memories and knowledge.

Yeah, it seems like Irminsul takes the path of least resistance during deletions, no matter what kind of inconsistencies or contradictions might arise. Rukkhadevata, as its administrator, could anticipate what that would look like and prepare accordingly; Scaramouche, as a depressed puppet, didn’t even know about the whole “it can only change information, not physical reality (aside from information *stored* in physical media like books, art, etc.)” deal, much less the possibility of plot holes.

The harbingers are not solely ranked by combat power. Only Childe thinks this is the case. Arlecchino fully believes she can kill Dottore anytime back in Fontaine and she isn't someone who overestimates herself or underestimates her targets.
They're more likely ranked by a combination of factors such as the order of joining, power and influence, and in Arlecchino's case, she just inherited the rank after proving to be much stronger than the previous number 4 as like a 14 year old.
